Friends & Family Plan

The Friends and Family Plan offers:

✓ Immediate eligibility (no waiting periods)

✓ No claim forms

✓ No pre-authorization requirements

✓ No pre-existing condition exclusions

Plan Fees:

Individual annual plan fee is $399 ($349 for each additional family member).

Since our exclusive Friends and Family Plan is not insurance, all applicants are guaranteed acceptance.

You will not need a membership card – your plan is effective as soon as you enroll. Enrollment is open after your initial appointment.

*With the Friends & Family Plan, you save $204 per year! Plus all the additional discounts on any other procedures just for having the plan.

Guidelines

• • The plan applies only to services provided at North Valdosta Dental Care.

• • The plan fees are non-refundable.

• • The plan is intended for patients without dental insurance. Patients cannot use both dental insurance benefits and discounts from the plan.

• • The plan cannot be transferred to another patient.

• • Use of Care-Credit to pay for the plan and finance treatment will require a
    convenience fee. Please see the front office for details.

• • For patients with gum disease, Periodontal Maintenance visits instead of
    regular cleanings will be covered for 2 of those visits per year, and will
    receive 25% discounts on any additional Periodontal Maintenance visits
    for the year.

• • Cosmetic procedures not included.
